| You will be required to call into the practice bringing with you your
NHS medical card if you have one. If you do not have you medical card
you will need to complete a registration form. A new patient
questionnaire for each member of the family wishing to register will
also require completion. A registration pack will be given to you, which will include a
copy of the practice booklet, details of the out of hours service, NHS
Direct and a health information booklet.
A health screening is included in the registration process. This is
undertaken by a practice nurse and should take about 15 minutes
including a check of your blood pressure, weight, height and urine. You
will be asked to provide relevant information about your health
background.
Change of Address
If you are registered at the surgery and change your name,
title, address or telephone number, please print off the form
below and return it to the surgery.
Any changes to personal details are subject to approval by the
Headlands Surgery. For example, a change of address must be
within our practice boundary and a change of name for a child
would require legal documentation.
