You will be required to call into the practice bringing with you your NHS medical card if you have one. If you do not have you medical card you will need to complete a registration form. You will also be required to produce evidence of identity ie passport, driving licence, official identity card or birth certificate or marriage certificate and evidence of address ie council tax bill, utility bill (gas or electric), bank statement, tenancy agreement. These documents must be dated within the last 3 months. Everyone over the age of 16 has to attend a new patient interview and a new patient questionnaire for each member of the family wishing to register will also require completion. Please also produce appropriate documentation to evidence change of name.

A registration pack will be given to you, which will include a copy of the practice booklet, details of the out of hours service and NHS Direct.

A health screening is included in the registration process. This is undertaken by a practice nurse and should take about 15 minutes including a check of your blood pressure, weight, height and urine. You will be asked to provide relevant information about your health background.

Change of Address

If you are registered at the surgery and change your name, title, address or telephone number, please print off the form below and return it to the surgery.

Any changes to personal details are subject to approval by the Headlands Surgery. For example, a change of address must be within our practice boundary and a change of name for a child would require legal documentation.

Disabled Access
We have a ramp to the front entrance, automated entrance doors and a wheelchair on the premises for those who require it.  There is also a toilet on the ground floor which is accessible by wheelchair and one in the health visitors' suite.  A hearing loop is installed in the practice.

If access should prove difficult, or assistance is required, please let us know of any suggestions you may have for us to consider.

Security
For security purposes some parts of the building (not consulting rooms) are covered by CCTV.
